Documentation Request Denials

CARC 163

The payer needs medical records before it will adjudicate the claim.

What it is

The pattern

A documentation denial is a hold, not a rejection: the payer needs medical records (operative notes, progress notes, orders) before it will adjudicate. The claim is suspended pending the records, and the window to send them is usually short.

Why it happens

Common causes

  • The claim tripped a medical-review edit and the payer needs the record to complete adjudication.
  • The documentation did not support a code that required it.
  • The request went to the wrong address or the fax failed silently.

Prevention

Stop it before the claim goes out

  • Keep a release-of-records workflow ready: the same day a documentation request arrives, the record goes out.
  • Track the payer's response deadline — missing it converts a hold into a denial.
  • Include the claim number and DOS on every records page.

When it happens

What to do

Send the exact records requested, with the claim number on every page, by the deadline — and get proof of delivery. If the records were sent on time and the claim was still denied, appeal with the delivery proof and a copy of what you sent.

The payer requested medical records, orders, notes, certificates, or other documentation before it will process the claim.
